    The purpose of this study was to develop a regenerative therapy for the middle ear mucosa to prevent recurrence of middle ear cholesteatoma. In this study, we used a unique animal model with middle ear mucosal defects to investigate the regenerative effects of retinoids on the middle ear mucosa. As a result, local administration of retinoids regenerated middle ear lineage epithelial cells with characteristics histologically similar to those of normal cells. In addition, the regenerated lineage epithelial cells functionally had motility similar to that of normal mucosa. We also evaluated the inner ear toxicity of retinoids and found no obvious inner ear toxicity in this study. Retinoids are already widely used in clinical practice, and clinical trials in humans will be conducted in the future for clinical application.

    A model of middle ear mucosal damage was established using guinea pigs. Via transcanal approach using a 1.9 mm rigid endoscope under general anesthesia, the middle ear mucosa in the ciliated epithelial region around the Eustachian tube was scraped after opening the anterior tympanic membrane. Histological evaluation of the mucosal damage site by HE staining at 2 weeks postoperatively showed that the ciliated epithelial cells had disappeared and the normal mucosa had not regenerated in the mucosal damage model.
    We attempted to regenerate the mucosa of the middle ear by topically administering high-mobility group box 1 (HMGB1), a molecule that acts in tissue repair through induction of autologous bone marrow mesenchymal stromal cells, but found increased abnormal granulation tissue and submucosal bone thickening at the site of mucosal damage, suggesting that HMGB1 rather promotes an inflammatory response.

    Auditory steady-state response (ASSR) is a frequency-specific response. It is used to objectively evaluate the hearing level in sleeping young infants. To investigate the influence of sleep stage on ASSR, we measured the thresholds of ASSR and recorded electroencephalographic data to determine the sleep stage simultaneously. The thresholds of ASSR were especially lower in stage N3 (deep sleep) and REM (rapid eye movement sleep) Hence, sleep stage should be taken into account when estimating the hearing level from ASSR. Simultaneous measurements of ASSR and sleep stage are very difficult and complicated using commercially available equipment. Thus we are developing the new system to measure both of ASSR and sleep stage and to estimate the hearing level form ASSR thresholds with revision by sleep stage.

    We analyzed the ASSR signal in contrast to the background noise at different sleep stages. The noise level was especially low in stage N3 (deep sleep) and REM (rapid eye movement sleep), whereas there was no significant difference in the response signals among sleep stages. Consequently, the signal-to-noise ratio (SNR) was high in both stage N2, N3 and REM. Hence, sleep stage should be taken into account when estimating the hearing level fromASSR.

    Inner ear regeneration is one of the unsolved themes in therapeutic research for hearing impairment. In this study, we challenged the development of a new therapeutic method targeting auditory nerve-inner hair cell synapses based on the new pathological concepts of primary neural degeneration and cochlear synaptopathy.
    Rho-associated coiled-coil containing protein kinase (ROCK) inhibitors has been reported to have neuroprotective and regenerative effects on synaptic pathways. Therefore, we analyzed the effect of ROCK inhibitions in a model of peripheral axonal damage in the spiral ganglion neurons induced by the glutamate agonists, N-methyl-D-aspartate (NMDA) and kainic acid in the explanted cochlea.
    This study suggested that ROCK inhibitors would be a potentially therapeutic candidate for the regeneration of peripheral axons and synaptic reformation in the cochlea.

    We confirmed many advantages such as stable inner ear injury, sufficient weight gain, and no death after administration of gentamicin (40 mg / ml) to the guinea pig tympanic cavity. In addition, it was confirmed that the tissue damage was hair cells only, and the tissue morphology of the surrounding environment such as supporting cells was maintained. In these models, MUSE cells were administered, and at 2 weeks, it was confirmed that the transplanted cells were engrafted in vestibular tissue.

    We examined the possibility of functional regeneration by transplanting Muse cells,a type of the pluripotent stem cell, to the injured inner ear in this study. Inner ear regeneration mechanism of Muse cells engrafted in the cochlea differentiate into inner ear cell lineage by their specific microenvironment and self repair of inner ear hair cells and synapses by trophic factor of Muse cells were analyzed in multiple aspects functionally, morphologically and immunohistologically using a guinea pig inner ear injury model and mouse inner ear three-dimensional organ culture.From this study, possibility of improvement of hearing by Muse cells in the guinea pig inner ear injury model was shown and Muse cells themselves have the possibility of differentiating to nerves in inner ear three-dimensional organ culture.

  • Regeneration of cochlear hair cell by transplantation of Multilineage-differentiating Stress Enduring (Muse) cell,2015.04 - 2017.03,Regeneration of cochlear hair cell by transplantation of Multilineage-differentiating Stress Enduring (Muse) cell

    First I made a inner ear degeneration model by band noise exposure. Noise frequency was from 4 to 8 kHz and its sound presser was 130 dBSPL. This condition can make a permanent threshold shift of hearing. I transplanted 10,000 Muse cells into scala tympani of this model using micro catheter 1 day after noise exposure. the hearing threshold recovered significantly on Muse transplanted ear at the area of 4 and 8 kHz from 6 weeks after noise exposure to the end point.I counted the rest of outer hair cell at 12 weeks after noise exposure, this revealed that more outer hair cells were alive in Muse transplanted group than control at 8 kHz area with a statistically significant difference. Now we are investigating whether Muse cells can migrate and engraft to the damaged organ of Corti and differentiate into hair cells or hair cell progenitor cells.

    Approximately 250-350g, male Hartley strain guinea pigs were exposed to 4-8 kHz octave-band noise at 136 dB sound pressure level (SPL) for 2 hours in the sound proof box under general anesthesia. All the animals were measured their ABR threshold before and 24 hours, 1 week and 2 weeks after the noise exposure to confirm the permanent threshold shift. The ABR threshold of 2 weeks after the noise exposure showed profound deafness. Cochlear frequency mapping was performed with ImageJ plugin to identify the tonotopic area, demonstrating that outer hair cells were disappeared.
    Multilineage-differentiating stress-enduring (Muse) cells were transplanted into the Scala tympani of the severely damaged animals of the number of 10,000 or 30,000 cells through a micro catheter. ABR of the transfected ears did not show evident hearing improvement compared to the control group. Immunohistochemistrical data show no GFP-positive cells alive in the cochlear.

    Objective: To determine the effect of transplanting normal human dermal fibroblasts (NHDF) in the rat facial nerve palsy model. Methods: Eight weeks old Wister male rats (250g) were used. Facial nerves were transected at buccal branch and marginal branch in 6 mm length under general anesthesia. Polimer tubes filled with NHDF plus matrigel were grafted between the cut nerves. Wisking amplitude was measured to determine the facial nerve function. Result: Wisking amplitude was gradually recovered in 8 week. However, the effect of NHDF was not significant.

    Diacylglycerol (DG) is one of the important second messengers, which serves as an activator of protein kinase C (PKC). DG kinase (DGK) phosphorylates DG to generate phosphatidic acid, thus DGK is considered to be a regulator of PKC activity through attenuation of DG. Recent studies have revealed molecular structures of several DGK isozymes from mammalian species, and showed that most of the isozymes are expressed ih the brain in various amounts. Five DGK isozyme cDNA have been cloned from rat brain library (DGKα, -β, -γ, -ζ, -ι) in Department of Anatomy and cell biology, Yamagata University School of medicine. mRNA expressions of DGK isozymes in rat brain have been studied by in situ hybridization histochemistry, but their mRNA expressions in human malignant tumor cells, especially epithelial cells, have not been examined. In this study, we examined the mRNA expression of DGK isozymes in human squamous cell carcinoma cell lines. To study the localization and functional role of DGKζ, we generated the specific antibody against rat DGKζ and examined the specificity of the antibody. Furthermore, human malignant tumor cell has been cultured with DGK inhibitors and it was revealed that DGK inhibitor might suppress cell growth. It shows that DGK may have some relations to the molecular mechanism of cell growth.

  • Clinical Application of Auditory Steady-State Response evoked by multiple simultaneous amplitude-modulated stimuli (amplitude-modulation following response : AMFR) for Assessment of hearing in infants and young children,2001 - 2002,Clinical Application of Auditory Steady-State Response evoked by multiple simultaneous amplitude-modulated stimuli (amplitude-modulation following response : AMFR) for Assessment of hearing in infants and young children

    In recent years newborn hearing screening has been widely implemented using automated ABR screener and EOAE test unit. However, hearing threshold prediction and follow-up evaluation using traditional ABR technique cannot provide sufficient information in a frequency-specific manner. Thus, it is necessary to develop new tests to obtain detailed information from specific cochlear regions. Our study is indicated about new detection algorithm to use multiple simultaneous amplitude-modulated stimuli and separate analysis for each MF with phase coherence. The responses for each MF are separated with software-triggering from the averaged waveforms to obtain using simultaneously multiple amplitude-modulated tones, from which the phase coherence value is calculated for each MF. Test reliability of SSR using simultaneously multiple amplitude-modulated stimuli (Four-CF combined stimulus in MFs around 40 HZ) with our detection algorithm was evaluated among normal-hearing adults and comparison with resulting in mono AM tone was performed. In addition examples of puretone audiograms and thresholds estimated from SSR using simultaneously multiple ampliude-modulated stimuli were demonstrated in adult subjects with hearing impairment. The new algorithm for response detection of SSR using simultaneously multiple amplitude-modulated stimuli at the optimal modulation rates is expected in clinical use for assessment of hearing in infants and young children.
